Insurance Claim Instructions:

Once an individual player is registered with a team through the online registration process and paperwork has been submitted, Development Academy Athletes are insured through Health Special Risk Inc. We require the athlete to use their own or parent’s insurance as primary coverage and secondary insurance if applicable and U.S. Soccer Development Academy’s as the secondary or tertiary insurance. Please provide the hospital or doctor’s office with the insurance information found on both the primary coverage as well as U.S. Soccer’s Development Academy coverage (if being used as secondary insurance) to avoid billing issues. If the athlete does not have any medical insurance, U.S. Soccer’s insurance becomes the primary coverage. Those families holding a primary insurance policy should refer to your explanation of benefits before filling with HSR or seeing a doctor.

The procedure for filing is as follows:

  • Using the HSR Claim Form, please complete in full and fax to:

  • We will then be sure the completed report reaches HSR and the insurance process will begin to cover the sport related injury.

PLEASE REMEMBER THAT ALL CLAIMS MUST BE FILED WITHIN 48 HOURS OF THE INJURY.

Claims are for sports injury related accidents. The policies do not cover medical health related issues. If a player has other insurance, he is obligated to carry his own medical health insurance and is responsible for any billing related to such.
