DEMOSPHERE SYSTEM:

Online Registration instructions can be found at http://ussda.demosphere.com/howto/

The Club Contact is a person from the club who can access both Academy team’s information. The club contact can sign-up additional team managers and coaches to assist with the online registration, club contact information and schedule. Club Administrators must log in at: http://ussda.demosphere.com/clubadmin  

Coaches and Managers that are not assigned to Club Administrative access may access their log in at: http://ussda.demosphere.com and click on the team page link at the top right corner.

INFORMATION TO BE SUBMITTED ELECTRONICALLY:

Teams should have the following components ready to access Demosphere:

  1. Player head shots
    Each player will need a head shot in a jpg format that can be uploaded to the Demosphere system. The photo will appear on the player’s profile and ID Card.
  2. Player Primary Insurance Information
    Copies of insurance cards should be collected from each player. This information will be a required step in #4 below.
  3. Academy Club Personnel Background Checks
    Background checks will need to be completed every other year. Personnel new to the Academy should complete background checks this year by following the instructions posted at http://www.ussoccer.com/common/stContent.jsp_168-DevAcadBGChecks.html.
  4. Players Insurance Information
    http://ussda.demosphere.com/insurance. Each team is responsible for inserting the player insurance policy information. Should insurance information not be available, contact Melissa Biniewicz at mbiniewicz@ussoccer.org  

PAPERWORK TO BE MAILED TO U.S. SOCCER:

The following documents will need to submitted via mail to U.S. Soccer.

  1. Waiver & Indemnification Form
    Every participant will need to complete a new waiver each year. The waiver form can be found at http://images.ussoccer.com/Documents/cms/ussf/Development%20Academy%20Waiver%20Indemnification.pdf
  2. Proof of age Documentation
    Previous proof of age paperwork will be kept on file for the following seasons. New players to the Development Academy must submit one of the following documents to demonstrate proof of age: birth certificate, driver’s license or passport.
  3. Developmental Player Documentation
    Developmental players added to an Academy roster on or after January 1, 2009 will need proof of registration with the Academy club at some point between July 23, 2008 and December 31, 2008. Note: Development Players will be ruled ineligible in the Academy if they are registered for two different clubs at the same time during this window.

Instructions for Submitting Paperwork

  1. Please print off a master roster from the online system and place it on top of the paperwork.
  2. Waivers and proof of age documents should be placed together for each player, in alphabetical order. If proof of age has previously been submitted, please indicate by putting check mark on the top right corner of the waiver form.
  3. If a player is to be designated as a developmental player, please put “DP” on the top left corner of the waiver form.

Approval of Players
Melissa Biniewicz will approve each player on a team’s roster before they are made eligible on match reports and weekly rosters. Please be sure all online fields are completed and the paperwork is submitted for her to be able to approve all players.

MEDICAL WAIVERS
After consultation with U.S. Soccer Legal Staff, Academy clubs will continue to be responsible for collecting and having access to players medical release forms.

ID CARDS
Presenting passes prior to games is a requirement for Academy participants. The 2008-2009 passes will be issued before a team's first game to the Club Contact pending receipt of the proper online and hard copy information.

Should a player lose a card, a $10 fee will be assessed to the club and the card will be sent the following week.

INTERNATIONAL CLEARANCE:
If a player was born outside the United States or left the United States, he will need International Clearance. Submitting birth documents to Melissa Biniewicz triggers the clearance process and will not begin until received from the Club Administrator or Manager. A clearance email from Melissa Biniewicz will conclude the clearance process and confirm eligibility for Academy participation.

  1. Proof of Entry Prior to 12
    If for any of the players below, you can show U.S. Soccer clear proof (School records, immunization records, Green Cards, etc.) that shows the player entered the United States prior to 12 (therefore
    should have players name and a date on it that shows it was issued prior to age 12), he will be able to clear the player automatically upon approval from Melissa Biniewicz (via email to the Club Admin).
  2. Waiver Process
    For players that are currently 12-16 years old, and are not eligible for or cannot show Proof of Entry Prior to 12, please fill out the attached Waiver Form. Have the form signed by player and parents and
    then send it to Steven Murry’s (U.S. Soccer Registration Department) attention along with a check made payable to US Soccer in the amount of $10.00 per Waiver.

    For this process, Steven Murry will need the following provided:
    1. Waiver Form - Filled out completely and signed by player and parents (we
      will act as the State Association for signature purposes).
    2. Check made payable to US Soccer in the amount of $10/waiver.
  3. ITC Process - Players Currently 17 Years Old
    1. Please fill out the ITC Request Form. This form needs to be signed by both the player and parents.
    2. Steven Murry will also need a letter from the player's parents stating that they
      have moved to the United States for "non-footballing" reasons. The letter needs to come from a parent stating when and why they moved to the United States. This is to allow us to exercise an exception to FIFA's rule prohibiting the transfer of players under the age of 18. The exception that applies in this case is that players may move to a new country and have an ITC issued, if they have moved to the new country with their family and the family has moved for "non-soccer related" issues. Please have the parent write/type out a brief, one or two paragraphs, letter stating such.

Once Steven Murry receives all required information, U.S. Soccer will request the ITC from the former Federation. They have up to 30 days to respond, but generally, most
federations get this process completed within a week or two.

For this process, Steve will need the following provided:

  1. Fill out an ITC Request Form
  2. Letter of Non-Footballing Reasons provided by parent/guardian
  1. ITC Process - Players Currently 18 Years Old and Above
    Please fill out the ITC Request Form and fax it to Steve's
    attention at 312-808-9263. This is all that is required for this process.
  2. Previous Clearance
    Players who have been previously cleared should let Steven Murry know so he may check the database of clearance. If the player has left the United States since the date of clearance, he will have to go through the clearance process according to the above information.
